A group of doctors, hospitals or other providers that provide heath care services at a reduced fee for members. If the member goes "out of network", they will pay higher fees than if they stay within the Preferred Organization. For a current list of Preferred Providers, visit the Aetna website.
Currently 145 hours times the current rate is needed each month in order to maintain eligibility.
Your remaining coverage can be determined by taking the dollar amount that is in your reserve and dividing it by the current contribution rate of each month. This will tell you how many more months your reserve will carry you.
You will become eligible on the first day of the second month following the month in which 240 paid work hours are credited to your Dollar Bank account.
Eligibility under the medical plan includes coverage for both vision and prescription drug benefits.
If you become inactive for twelve consecutive months with no activity on your account, your Dollar Bank reserves will revert back to the Trust and you must re-establish initial eligibility.

The Trust offers a 24 Hour Nurseline. Please call 1-866-886-0633 ANYTIME to receive assistance with any questions regarding your immediate health care. Registered nurses will answer your questions and refer you to a physician, minor emergency facility or emergency room if needed.

Pension related information

First-time users can create a User ID (which replaces the use of your Social Security Number), by clicking on the Establish User ID and PIN/Password link on the John Hancock Retirement Plan Services website. If you already have a PIN you can create a User ID by visiting the User Profile page located under the Manage My Account tab. To access your secure retirement plan account, enter your Social Security Number, or User ID (if you have created one) and your Personal Identification Number (PIN)/Password. If you have previously created a User ID, you must use your User ID to log in.
